Your Oasis Hospice Team
Oasis Hospice has provided the highest quality care to patients and families living with advanced illness, while honoring human dignity and personal choice. Oasis strives to treat the whole person — fulfilling physical, psychosocial and spiritual needs — and views our patients and their loved ones as a unit.
Patient and Family: The patient and their family play a crucial role in the hospice team. We will work closely with you and your loved ones to identify and prioritize the goals that matter most to all of you in your care plan.
The Nursing Staff: Consisting of skilled and experienced Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs), they deliver hands-on care, assess patient needs, and manage pain and symptoms. The Nursing Team offers support and education to family members on how to care for their loved ones.
The Medical Team: Led by Medical Directors and the patient’s primary care physician, our doctors develop a care plan to manage pain and control symptoms related to the patient’s terminal illness or disease.
Hospice Aides, including Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs): Assist with personal care, daily activities, and provide companionship and emotional support to patients and families.
Social Workers: Help patients and families address communication, conflict resolution, and other stressful situations, while coordinating community resources and assisting with non-medical matters such as finances and end-of-life concerns.
Chaplains and Spiritual Care Coordinators: Offer spiritual support, guide patients and families through end-of-life questions and decisions, and help to restore family relationships.
Bereavement Coordinator & Volunteer Teams: Help patients and loved ones prepare for the end-of-life process, provide ongoing bereavement support and grief counseling for up to 13 months. Trained volunteers provide companionship, support, and assistance with caregiving responsibilities and household chores during periodic visits with patients.
