Setting Up Hospice
In Texas, establishing hospice care requires teamwork among the hospice provider, patient, family, and healthcare team to deliver compassionate and thorough care that respects the patient's desires and offers solace and support in the last moments of life.

Reach out to a hospice provider, such as Oasis Hospice, to discuss the needs of the patient. -
Referral:
A referral can be made by the patient’s primary care physician, a hospital, or other healthcare providers. Family members can also initiate contact directly with the hospice provider.

Medical Evaluation:
The hospice team reviews medical records and will conduct an evaluation to assess eligibility for hospice care, usually for patients with life-limiting conditions. -
Certification:
